For verification and accuracy, documents containing the 15-digit microchip number MUST be submitted with your microchip registration form.

What if I don’t have proof of purchase or documents for my pet’s microchip? ❓🤔❓

Answer:

If you don’t have a purchase agreement or any documents that show your pet’s 15-digit microchip number, don’t worry — we can still help you register your pet.

Here’s what to do:

✅ Step 1 – Get the Microchip Number Scanned

Visit your veterinarian and ask them to scan your pet’s microchip. Request a document or written note that includes:

  • The 15-digit microchip number

  • Your pet’s name

  • The vet clinic’s name and date of scan

You can upload a photo or PDF of this document to us as part of your registration.

✍️ Step 2 – Submit a Signed Statement

If you don’t have proof of purchase or adoption, we also ask for a signed statement confirming that you’re the legal owner.

You can write this yourself or use the template below:

📝 Sample Statement:
I, [Your Full Name], confirm that I am the legal owner of the pet associated with microchip number [15-digit number]. I do not have a purchase agreement or supporting documents, but I have verified the microchip through my veterinarian and request that it be registered in my name. I affirm that this submission is true and made in good faith.
