For verification and accuracy, documents containing the 15-digit microchip number MUST be submitted with your microchip registration form.

If you don’t have a purchase agreement or any documents that show your pet’s 15-digit microchip number, don’t worry — we can still help you register your pet.
Here’s what to do:
✅ Step 1 – Get the Microchip Number ScannedVisit your veterinarian and ask them to scan your pet’s microchip. Request a document or written note that includes:
The 15-digit microchip number
Your pet’s name
The vet clinic’s name and date of scan
You can upload a photo or PDF of this document to us as part of your registration.
✍️ Step 2 – Submit a Signed StatementIf you don’t have proof of purchase or adoption, we also ask for a signed statement confirming that you’re the legal owner.
