If this is your first time registering your pet's microchip or you're unsure about the registration form, please follow the instructions below for a step-by-step guide on how to complete the registration.
This is your pet’s unique ID. Make sure it’s correct — it links your contact info to your pet.
State whether you’re a:
- Pet Owner
- Breeder
- Rescue/Shelter
Upload one or both of the following:
A photo showing the 15-digit microchip number (e.g., from the tag or sticker)
Proof of ownership, such as:
Vet records
Bill of sale
Adoption paperwork
Documents must include the 15-digit microchip number and help support the request. Please note that microchip registration is an administrative record only and should not be relied on as conclusive proof of legal ownership.
