If this is your first time registering your pet's microchip or you're unsure about the registration form, please follow the instructions below for a step-by-step guide on how to complete the registration.
Step-by-Step Guide to Registering Your Pet's Microchip:
Microchip Number (15-digits)
- This is the unique identifier associated with your pet. Ensure this number is correct as it is used to link your contact details with the pet.
State if you are a PET OWNER / BREEDER / RESCUE
Upload supporting documents:
- A photo of the 15-digit microchip number (tag, sticker) AND proof of ownership (bill of sale or vet records). Multiple files must be uploaded at the same time - MAXIMUM 2 FILES (JPEG OR PDF FILES).
Owner Information:
- Email: Enter a valid email address for communication purposes.
- Full Name: Provide your legal name for identification.
- Phone Number: This is the primary contact method if your pet is found.
- Street Address: Providing your street address ensures your ownership can be verified by authorities if identification is ever required.
Pet Details:
- Animal Name: Enter the name of your pet as you would like it displayed in the system.
- Animal Birthday: This helps track your pet's age.
- Species: Choose whether your pet is a cat, dog, or another type of animal.
- Breed: Provide the breed of your pet (or breed likeness).
- Color of the Animal: State the predominant colour.
Consent Statement:
- Ensure you read and provide consent for storing and using this data in case of emergency or when it becomes necessary to retrieve information. Tick the box is your agree.
Submit the Form:
- Once all the necessary fields are completed and read, click Submit.
- You will receive a confirmation email that the registration has been successful - please allow 2-3 business days.
